Registration Forms and Fees
Registration Check List - All items listed below MUST be SIGNED and SUBMITTED before you step on the ice.
R Registration Form (One copy completed and SIGNED at bottom)
R Full Fees (One cheque for full amount or two cheques with one postdated - following the fee schedule)
R ORA Membership Form (TWO copies of form completed and SIGNED in the box at the right - both copies are on the one PDF sheet )
R Player Code of Conduct (One copy completed and signed)
R Shirt Deposit Cheque ($100 cheque post-dated to April 1st, 2011)
Coaching Application - Open A, B+, B-, C
